Health Requirements for Incoming Students

所有病史, 免疫接种, and student-athlete information needs to be completed by July 30th in order to access your dorm room and participate in fall athletics. 

I. 病史:

(Required for all 本科 and 研究生 students)

All undergraduate and graduate first-year and transfer students are required to submit a medical history. Medical information is strictly confidential and is used to provide the appropriate care for the student when needed. You can complete your medical history form by accessing your 地中海+普氏 账户.

II. 免疫接种:

(Required for all 本科 and 研究生 students)

Manchester University requires all entering students to present evidence of immunity to certain diseases before they may arrive on campus. Students whose religious beliefs conflict must fill out the religious exemption form located on your 地中海+普氏 .

Required 免疫接种s are:

• MMR-Measles, Mumps, Rubella (series of two)
• Meningococcal or Menactra or MCV4- (series of two)
• Meningococcal B (series of two)
• TDap/tetanus/diphtheria (within the past 10 years)
  

For International, Advanced Nursing, 研究生 and Doctoral students ONLY: 

• Tuberculosis screening test or QuantiFERON Gold Blood test (must be less than 12 months old)
  

Recommended but not required 免疫接种s are:

• Covid-19疫苗接种
• Flu Vaccination (REQUIRED FOR PharmD & ABSN学生)
  
  

3. Emergency Medical 联系 Information:

(Required for all 本科 and graduate students)

All incoming students are required to complete the Emergency Medical 联系 Information using their MU login information. The form can be completed on our ChetNet site at the following link:

Students need to complete this form within their first week on campus each year.

IV. 体格检查

(A physical exam is required for students participating in intercollegiate sports only)

Students planning to participate in intercollegiate sports must have a physical within the past 6-months from the start of the academic school year for those students enrolling in the fall. For those students enrolling in the spring, sport physicals must be within 6-months from the start of the spring semester. If you do not plan on participating in intercollegiate athletics there is no need to get a physical examination.

V. Sickle Cell Trait Status

The NCAA is mandating that its member institutions verify sickle cell trait status of every student-athlete beginning August 1, 2022. Although there are no requirements limiting participation in sports by student-athletes who have the sickle cell trait, the NCAA requires Athletic Departments to identify each athlete’s status to better educate and identify high-risk student-athletes. All student-athletes at Manchester University are required to provide documentation demonstrating the presence or absence of sickle cell trait. Please have your healthcare provider complete this form by doing a sickle cell solubility test or by documenting results from a previous test.
